Patients who can ideally benefit from the Catastrophic Care Solutions program include those who have had surgery, illness or an injury that affected their spinal cord, rendering them chronically disabled.

As much as possible, we will maximize their recovery potential and will teach them skills to cope with their current condition. We also encourage them to regain their independence through:

FALL PREVENTION

Falls are the second leading cause of accidental death in the United States. Seventy-five percent of these falls occur in the older adult population. One third of the older adults who fall, sustain a hip fracture and are hospitalized, die within a year. Falls not only affect the quality of life of the individual but also influence the caregiver and family. Health care costs for falls and rehabilitation average 70 billion dollars a year!

Even if the fall does not result in hospitalization, fear of falling becomes a major factor. Fear leads to inactivity and loss of confidence. This, in turn produces a cycle of fear, loss of self-confidence, and inactivity, thereby decreasing the quality of life and increasing the risk of falls.

Some health care individuals state that only one-third of the community-dwelling older adults fall each year, which is a relatively small number. Therefore they question the expenditure of the dollars for fall prevention. However, the argument can be countered by the fact that fall prevention programs may preserve and possibly improve the quality of life for even one individual. Second, we do not know the long-term health care benefits and reduction in costs for the general population. Some studies are beginning to demonstrate a cost-savings; that is, if the older adults are provided with a series of educational programs and environmental modifications, then the health care costs are reduced. Our knowledge as health care professionals may be able to reduce falls through volunteer efforts. The improving of one person's quality of life and possibly decreasing fall-risk factors is worth the time. The following information is by no means either a comprehensive or a "sure-fire" plan to prevent falls, but it is one possible method to help increase awareness regarding falls and fall prevention.

PULMONARY REHABILITATION

Pulmonary rehabilitation is a program to help people with chronic lung diseases like COPD, emphysema, and chronic bronchitis lead full, satisfying lives and restore them to their highest functional capacity. Pulmonary rehab is aimed to improve quality of life by:

  • Encouraging self-management and control over daily functioning

Anrex's team approach to pulmonary rehab

Most formal pulmonary rehab programs include a team of healthcare providers working together, like doctors, nurses, rehabilitation therapists, psychosocial staff and dietitians. The team evaluates each person's overall physical and emotional status, and then develops an individual program for him or her. The doctor, a specially trained registered nurse, a rehabilitation specialist or a respiratory care practitioner coordinate the program.

We also provide educational programs critiqued to best benefit the patient. A team specifically dedicated to wound care with clinical nurses who specialize in wound care.

Our Pressure Ulcer Prevention Program (PUPP), Compass and Hand Hygiene programs are designed to address and meet the challenges of pressure ulcer prevention, wound care and hand hygiene compliance. We have made updates to our Advanced Wound Care packaging to provide an improved delivery and communication system that helps the family and patient better understand and more easily deliver wound care at the patient's bedside. The packaging itself serves as a two-minute course on advanced wound care. Our programs provide the ability to constantly care for wounds properly.

Skilled Home Health Care

Skilled Home Health is short-term health care services that can be provided to people if they are homebound. Skilled Home Health Care is for people who need skilled services, case management, and assistance with activities of daily living.

Skilled Home Health Care can be used in combination with other Home and Community Based Services. Based on individual need and resources.

Who is eligible for Home Health Services… Are you Homebound? Are you at risk for a fall? Are you medically unstable and, under the care of a physician? Has your medication and/or medical condition recently changed? Were you recently discharged from the Hospital or Nursing home and need medical assistance at home to avoid re-hospitalization:

We will work with you to evaluate the level of care that is necessary to improve your loved one’s quality of life. We offer a variety of services to give our clients the specific care they need, including:

  • Registered Nurse-will complete a thorough assessment to determine how the individual needs can best be met. A care plan is formulated with input from the client. Goals and objectives are clearly defined. The RN serves as a case manager working closely with your physician and all assigned disciplines to improve your health well being.
  • Licensed Practical Nurses-are at times assigned to partner with with the registered nurse in providing hands on care under the RN’s supervision.
  • Physical Therapist(abbreviated PT)-provide comprehensive therapeutic modalities to promote healing, safety, strengthening pain control increasing mobility and independence.
  • Occupational Therapists-(also abbreviated as OT) Occupational therapy interventions focus on adapting the environment, modifying the task, teaching the skill, and educating the client/family in order to increase participation in and performance of daily activities.
  • Speech Therapy- focus on improving speech deficiencies, and swallowing deficiencies which enhance the ability to effectively communicate, improve the ability to maintain a healthy, nutritional balance and, gain independence at an optimal level.
  • Medical Social Workers – Medical social workers assess the psycho social functioning of patients and families and intervene as necessary. Interventions may include connecting patients and families to necessary resources and supports in the community; providing psychotherapy, supportive counseling, or grief counseling; or helping a patient to expand and strengthen their network of social supports.
  • Dietitians – Dietitians are experts in food and nutrition. They advise people on what to eat in order to lead a healthy lifestyle or achieve a specific health-related goal.
  • Home Health Aides and CNA’s &are trained to safely provide clients with the basic activities of daily living under the supervision of the Registered Nurse with a structured individualized care plan.
